KHEIR Center is committed to providing linguistically and culturally-sensitive quality primary healthcare and human services support to the underserved and uninsured residents of Southern California.
KHEIR Center was established in 1986 to serve as a bridge between low-income, recently-immigrated, non-English speaking Korean community and local health and social service providers. Today, KHEIR has evolved into a broad-based agency that provides services to the indigent population in four interrelated areas; Human Services, Senior Affordable Housing, Adult Day Heath Care, and Healthcare services.
Health Education & Outreach Specialist
Health Education & Outreach Specialist will conduct brief health education/awareness and outreach contacts and encounters. This person will educate potential clients about health coverage programs, enroll clients into the appropriate coverage programs, assist with applications, and provide follow-up to ensure enrollment, utilization, and retention. In addition, the Health Education & Outreach Specialist will make presentations to clients on specific health-related topics to increase awareness. This is a full-time position that requires field work.
Responsibilities:
• Provide outreach to explain KHEIR’s Clinical Services.
• Provide outreach to explain Medi-Cal, Healthy Families, Healthy Kids and other health care options available to parents and the general public at multiple off-site and on-site locations.
• Plan and conduct orientation meetings, trainings, workshops, and other outreach/informational events for child care providers, schools, and community based programs, shelters, churches, and other venues.
• Assist members in the enrollment process for health insurance benefits and services that the organization provides.
• Act as a resource of information on the phone or in person, for reliable information regarding the clinic’s programs.
• Schedule appointments for patients, and follow-up on any missed appointments
• Provide outreach, information, and referrals to adolescents for Clinic programs.
• Assist CHDP – Gateway patients to ensure that they are receiving all the services that are available.
• Facilitate presentations to community members and other CBO’s regarding Medi-Cal, Healthy Kids, Healthy Families, and other health care options available to parents.
• Facilitate presentations on other health-related topics including, but not limited to, teen pregnancy preventions and STD’s, importance of prenatal and pediatric care, etc.
• Maintain records, logs, signing sheets, and other documentation required to track for contract compliance and evaluate program performance.
• Other duties as assigned.Qualifications:
• Minimum 2 years experience in community outreach work
• College education preferred but not required
• Certified Application Assistant
• Bilingual; English/Spanish or English/Korean
• Flexible work schedule; may be required to work some nights and weekends
• Strong computer skills
